When it comes to your health, you want to make sure that you are making the best decisions for your care. However, what happens if you are unable to direct your care? With an advanced care directive, you can legally designate a person, known as an agent, who will have the power to make decisions on your behalf concerning your treatment and care. The directive may also specify the types of treatment, giving the agent guidance to the care you want employed or excluded.
What steps do we take in preparing an advanced care directive?
- First, we schedule an initial meeting, where we will review initial information to prepare your legal filing. At this meeting, you will provide the name of the person who is to be granted agent status.
- Second, our office will prepare a draft advanced care directive.
- Finally, we schedule a meeting so you can sign the advanced care directive.
The service fee for advanced health care directive is only $50 and includes all additional fees and court costs.
